- Do you have fever?
- Do you have any chronic health conditions that may put you at risk of infection?
- Do you have any health condition that lower your immunity?
- Did you travel out of the province in the last 14 days?
- Have you been in close contact with a person confirmed to having COVID 19?
- Have you tested positive for COVID 19?
